Short answer: On Kauai, two Lihue companies rent Jeeps set up for unpaved roads: Diff. (5.0 on 35 Google reviews) and Hawaii Lifted Jeep Rentals, which says its fleet is approved for unpaved state and county roads and parks. For paved roads only, a Jeep from a Lihue Airport company is the simpler choice.
Kauai is the island where a Jeep rental gets the most use, because a few popular spots sit at the end of dirt roads. That only helps if your rental agreement lets you drive there. Here are the Kauai companies that rent Jeeps and 4x4s, how they rate, and what each one allows.

Diff. rents 4×4 Jeeps and Broncos in Lihue and in Colorado. Its site describes the fleet as approved and equipped for off-road use, with all-terrain tires and air compressors for trails and beaches. Recent Kauai reviews on its site mention a taxi between the airport and its lot.
Hawaii Lifted Jeep Rentals
Hawaii Lifted Jeep Rentals is family-run from Kona and Lihue. It rents Wranglers, Rubicons, Gladiators, Wagoneers and Broncos, says every vehicle is approved for unpaved state and county roads and parks, and requires drivers to be 21 or older. Rates include unlimited miles.
Hawaiian Overlanders
Hawaiian Overlanders rents 4WD Toyota FJ Cruisers set up for camping, so the vehicle doubles as your bed. Its listings started at $177 a day when we checked.

Do you need an off-road Jeep on Kauai?
For most trips, no. The drive to Waimea Canyon, the north shore and Haena State Park is paved. A Jeep from a regular company is fine for those; Budget, for example, has a Lihue Jeep rental page. The off-road-approved companies matter only if your plan includes unpaved roads, because standard rental agreements do not cover them.
Pick the door count before the company. One Kauai Jeep renter, Discount Kauai Jeep Rental, lists 12.8 cubic feet of cargo space behind the rear seats in a 2-door Wrangler and 31.5 in a 4-door. Hawaii Lifted Jeep Rentals lists its 4-door Wranglers as holding 4 large suitcases.
